SLT Support

The form below is for children you would like to bring to the attention of the Sure Start South Armagh Speech and Language Therapy Team. Typically these will be children who have identified needs with their communication, and who have not yet been seen by the Core SLT Health Service team. Children must be registered with SureStart South Armagh (or eligible to) and it is essential you obtain parental consent before submitting this form.

For children and families you feel require wider support from our Family Support Team, please complete a TASK referral form. Speech, Language and Communication needs will be considered as part of the wider support offered by our service. There is no need to complete both forms.
